SUBCONTRACTOR REQUIREMENTS

DOCUMENTATION AND INSURANCE REQUIREMENTS:

  • Once awarded and prior to starting work, Subcontractor must execute and return the standard subcontract provided by M-PAK Construction, Inc.
  • Prior to starting work, Subcontractor must furnish a W-9
  • Prior to starting work, the Subcontractor shall obtain the required insurance from a responsible insurer, and shall furnish satisfactory evidence to M-PAK Construction, Inc.
  • Workers’ Compensation/Employers’ Liability Covering Each Accident/Incident OR Workers’ Compensation Wavier Applicable for each state where work is performed
  • Commercial/General Liability coverage requirements are as follows:
    • Per Each Occurrence: $1,000,000.00
    • Damage to Rented Premises: $100,000.00
    • Medical Expenses: $5,000.00
    • Personal and Advertising Injury: $1,000,000.00
    • General Aggregate: $2,000,000.00
    • Products: $1,000,000.00

 

  • Subcontractor must provide CONTRACTOR with certificates of insurance, evidencing Waivers of Subrogation in favor of MPAK Construction, Inc. on General Liability.
  • Subcontractor must name MPAK Construction, Inc. as a Certificate Holder AND an Additional Insured on General Liability
  • Any insurance policy must also provide for a 30-day notice of cancellation or a material change in coverage
  • All insurance required shall be maintained for the project duration and include Completed Operations for the Additional Insured until each party’s statutory liability has ceased. Such insurance shall be primary and noncontributory to any insurance maintained by M-PAK Construction, Inc.

APPLICATIONS FOR PAYMENT:

  • All Applications for Payment/Invoices must be emailed to the accounting@mpakconstruction.com
  • Applications for payment/Invoices shall be paid within Thirty (30) days of receipt of Invoice, once payment has been received by the project owner.
  • Retainage must be shown on the application for payment.
  • Executed conditional progress or final lien releases must be submitted before payment can be released.
  • All Change Orders must be approved in writing by one of CONTRACTOR’S project managers before work can begin.
